Reconstructive challenges in the extended endoscopic transclival approach

Objective:
We wanted to present our experience with the extended endoscopic approach to clival pathology, focusing on cerebrospinal fluid leak and reconstruction challenges.
Methods:We examined a consecutive series of 37 patients undergoing the extended endoscopic approach for skull base tumours, 9 patients with clival pathology. Patients were examined for the incidence of post-operative cerebrospinal fluid leak in relation to tumour pathology, location, size, reconstruction and lumbar drain.
Results:The overall incidence of post-operative cerebrospinal fluid leak was 10.8 per cent. Seventy-five per cent of patients who had a post-operative cerebrospinal fluid leak underwent a transclival approach (p < 0.05). All patients with clival pathology who underwent an intradural dissection had a post-operative cerebrospinal fluid leak (p < 0.05).
Conclusion:Post-operative cerebrospinal fluid leak rates after the extended endoscopic approach have improved significantly after advancements including the vascularised nasoseptal flap. Despite this, transclival approaches continue to pose much difficulty. Further investigation is necessary to develop technical improvements that can meet the unique challenges associated with this region.

We present distance determinations to the LMC and SMC based on long period eclipsing binary systems composed of giants. Our LMC distance provides the best zero point for the whole extragalactic distance scale, while the precise relative distance between the LMC and SMC offers unique opportunity to study populational effects on the stellar distance indicators. Apart from distances, analysis of these systems allowed us to measure with a very good accuracy basic stellar parameters (1–2% radii, masses, effective temperature, etc.) of relatively large sample of giants. This catalog provides unique opportunity to calibrate evolutionary models of giant stars.

We present a precise and accurate measurement of the distance to the Large Magellanic Cloud based on late-type eclipsing-binary systems. Our results provide curently the most accurate zero point for the extragalactic distance scale.

Accurate assessment of neonatal body composition is essential to studies investigating neonatal nutrition or developmental origins of obesity. Bioelectrical impedance analysis or bioimpedance analysis is inexpensive, non-invasive and portable, and is widely used in adults for the assessment of body composition. There are currently no prediction algorithms using bioimpedance analysis in neonates that have been directly validated against measurements of fat-free mass (FFM). The aim of the study was to evaluate the use of bioimpedance analysis for the estimation of FFM and percentage of body fat over the first 4 months of life in healthy infants born at term, and to compare these with estimations based on anthropometric measurements (weight and length) and with skinfolds. The present study was an observational study in seventy-seven infants. Body fat content of infants was assessed at birth, 6 weeks, 3 and 4·5 months of age by air displacement plethysmography, using the PEA POD body composition system. Bioimpedance analysis was performed at the same time and the data were used to develop and test prediction equations for FFM. The combination of weight+sex+length predicted FFM, with a bias of < 100 g and limits of agreement of 6–13 %. Before 3 months of age, bioimpedance analysis did not improve the prediction of FFM or body fat. At 3 and 4·5 months, the inclusion of impedance in prediction algorithms resulted in small improvements in prediction of FFM, reducing the bias to < 50 g and limits of agreement to < 9 %. Skinfold measurements performed poorly at all ages.

We present new near-infrared Ks photometry of the interesting Galactic globular cluster NGC 6441. The optical-NIR color-magnitude diagram shows evolutionary features that seem to agree with a canonical evolutionary framework. The K-band Period-Luminosity-Metallicity relation of RR Lyrae stars gives a distance estimate of 15.51±0.07 that is slightly larger previous estimates.

Perispinal analgesia for labour followed by patient-controlled infusion with bupivacaine and sufentanil: combined spinal–epidural vs. epidural analgesia alone

Summary
Background and objective: Combined spinal–epidural is an alternative technique to epidural analgesia for labour, but its benefits are not clearly identified.
Methods: A prospective, blinded, randomized study was undertaken involving 113 women attending a university hospital obstetric department. Analgesia was initiated with intrathecal bupivacaine 0.25% 1 mL + sufentanil 5 μg in the combined spinal–epidural group (n = 54), and with bupivacaine 0.125% + epinephrine 2.5 μg mL−1 + sufentanil 7.5 μg in the epidural group (n = 59). In both cases this was followed by patient-controlled epidural analgesia with bupivacaine 0.125% (+sufentanil 0.25 μg mL−1). Duration of labour, quality of analgesia and side-effects were compared between groups.
Results: In the combined spinal–epidural group, the onset of analgesia was faster (5 vs. 15 min, P < 0.001), the consumption of bupivacaine was lower (7.5 vs. 11.3 mg h−1, P = 0.003) and there was less unilateral analgesia (14.8% vs. 40.7%, P = 0.002) than in the epidural group. The characteristics of labour were similar in both groups. However, in the combined spinal–epidural group, there was a higher incidence of posterior presentation (25.9% vs. 10%, P = 0.03), pruritus (P < 0.001), hypotension (P = 0.002), somnolence (P = 0.01), nausea (P = 0.02) and one case of meningitis.
Conclusions: The combined spinal–epidural technique provided more effective analgesia during labour than epidural analgesia alone but offered no other advantage. It induced more adverse effects and this should be considered before routinely using the combined spinal–epidural technique.
Long-term persistence of Coxiella burnetii in the host after primary Q fever

After a primary infection Coxiella burnetii may persist covertly in animals and recrudesce at parturition to be shed in the products of conception and the milk. Similar latent persistence and recrudescence occurs in man: namely, infection of placenta, heart valve or mural endocardium, bone or liver. The numbers of organisms, their viability and cellular form, and the underlying organ sites of latent infection for the coxiella are obscure. During investigations of 29 patients with a chronic sequel to acute Q fever, the post-Q fever fatigue syndrome (QFS) [1–3], sensitive conventional and TaqMan-based PCR revealed low levels of C. burnetii DNA in blood mononuclear cells (5/29; 17%), thin needle liver biopsies (2/14; 14%) and, notably, in bone marrow aspirates (13/20; 65%). Irrespective of the ultimate significance of coxiella persistence for QFS, the detection of C. burnetii genomic DNA in bone marrow several years after a primary infection unveils a new pathological dimension for Q fever.

High explosives, blasting agents, propellants, and pyrotechnics are all metastable relative to reaction products and are termed energetic materials. They are thermodynamically unstable but the kinetics of decomposition at ambient conditions are sufficiently slow that they can be handled safely under controlled conditions. The ease with which an energetic material can be caused to undergo a violent reaction or detonation is called its sensitivity. Sensitivity tests for energetic materials are aimed at defining the response of the material to a specific situation, usually prompt shock initiation or a delayed reaction in an accident. The observed response is always due to a combination of the physical state and the molecular structure of the material. Modeling of any initiation process must consider both factors. The physical state of the material determines how and where the energy is deposited in the material. The molecular structure in the solid state determines the mechanism of decomposition of the material and the rate of energy release. Slower inherent reaction chemistry leads to longer reaction zones in detonation and inherently safer materials. Slower chemistry also requires hot spots involved in initiation to be hotter and to survive for longer periods of time. High thermal conductivity also leads to quenching of small hot spots and makes a material more difficult to initiate. Early endothermic decomposition chemistry also delays initiation by delaying heat release to support hot spot growth. The growth to violent reaction or detonation also depends on the nature of the early reaction products. If chemical intermediates are produced that drive further accelerating autocatalytic decomposition the initiation will grow rapidly to a violent reaction.
